Insurance and Fees

I accept Blue Cross and Blue Shield PPO, Blue Cross and Blue Shield Medicare Advantage PPO, and United (Optum) insurance. If you would like to use your BCBS or United insurance, please contact your representative to verify coverage details (i.e., copayment amount, coinsurance, deductible etc.) and inform me prior to your first appointment. I will bill BCBS or United directly for services provided.

I am currently considered an out of network provider for all insurance carriers except BCBS PPO, BCBS Medicare Advantage PPO, and United (Optum). If you do not have BCBS or United coverage and would like to use another type of insurance, I will provide you with paperwork that you can submit to your insurance provider for reimbursement. Insurance companies vary in what they will reimburse for an out of network provider, but my understanding is that they usually reimburse at a rate of 50-80%. I recommend contacting your insurance provider to verify coverage and reimbursement rates for an out of network psychologist before you start treatment. For example: “Psychotherapy with Patient and/or Family Member” (CPT Code 90837) by an “Out of Network Provider.” If you use that terminology, they should be able to provide specific information regarding out of network reimbursement.

For private pay or out of network insurance, my usual fee for an initial diagnostic evaluation or a 50 minute therapy session is $200.00. If you feel you cannot afford this rate, please talk to me about this as I am sometimes able to offer a reduced fee for special circumstances for a limited number of clients.
